FOX Rehabilitation is a private practice of full time Physical, Occupational and Speech Therapists. FOX was built

on the novel foundation of Geriatric House Calls. Patients are evaluated and treated one-on-one in their homes to

address functional limitations and safety at home and in the community. Patients do not have to be homebound or

have a skilled nursing need and FOX’s evidence-based clinical services are covered under Medicare Part B.

Founded in 1972, Monroe County Meals on Wheels is the only local home-delivered meal provider offering nutritious meals and a human connection to older adults, adults with disabilities, and homebound individuals within Monroe County, PA. We serve anyone who is over the age of 18 and unable to shop for and/or prepare meals for themselves due to a physical or mental health condition. Depending on need, preference, and location, clients may choose to receive either a weekly delivery of frozen meals or a daily delivery of a hot dinner and a cold bagged lunch. Meal cost depends on a client’s age, income, and expenses – sliding scale funding and a variety of other options are available.

All meals are delivered by volunteers, who pick up meals at our office Monday – Friday and deliver on an assigned route that takes about 1.5-2 hours and includes approximately 10-15 stops. Volunteers can choose to deliver as little as once a month or as often as every day. The application process is simple; we welcome individuals, organizations, and groups to help make a difference in the lives of our clients!

The Church of Saint Luke

The Parish Nursing Ministry of The Church of Saint Luke consists of two Parish Nurses and a mixed team of nurses, other health care providers, educators, and dedicated parishioners. Mindful of the connection between faith and health, the team reaches out to support those who are facing challenging health issues to provide support, comfort, prayer, and practical assistance. Assistance may come in the form of home visits, home communion, transportation assistance to medical appointments, individual office visits, help finding community resources, and bereavement support. Also, the team meets monthly to assess parish health needs and plan health education events. Although nurses are strongly encouraged to join our team, no specific education or background is required aside from a desire to serve God and the parish of Saint Luke.
