It is the culmination of local professionals who identified a gap in awareness and interconnectedness of resources that can benefit seniors.

Residential offers comprehensive Home Health, Hospice and Palliative Care services to individuals throughout Eastern Pennsylvania. We focus not only on your medical needs, but also on your emotional and spiritual well-being. Our multidisciplinary team—including physicians, nurse practitioners, nurses, social workers, spiritual care counselors, nursing assistants, music therapists, massage therapists, and bereavement support professionals—works collaboratively to enhance your quality of life, empower decision-making, and provide dedicated support for both patients and families. Hospice care is fully covered by traditional Medicare benefits and can be provided in the comfort of your own home.

At Mrs. Bush’s Personal Care Home we are dedicated to providing today’s seniors with service, assistance and companionship and have done so since 1983. Caring Transitions of East Stroudsburg is designed to minimize stress by utilizing specially trained professionals to oversee every detail of a senior's transition with compassion, including decluttering, organizing, packing, move management, unpacking, resettling, and cleanouts. The company also manages both in-home and online estate sales. The online estate sales are hosted on CTBIDS.com, the brand's widely popular auction platform, which can support its clients in liquidating unique and everyday treasures that many cherish.

FOX Rehabilitation is a private practice of full time Physical, Occupational and Speech Therapists. FOX was built
on the novel foundation of Geriatric House Calls. Patients are evaluated and treated one-on-one in their homes to
address functional limitations and safety at home and in the community. Patients do not have to be homebound or
have a skilled nursing need and FOX’s evidence-based clinical services are covered under Medicare Part B.
Founded in 1972, Monroe County Meals on Wheels is the only local home-delivered meal provider offering nutritious meals and a human connection to older adults, adults with disabilities, and homebound individuals within Monroe County, PA. We serve anyone who is over the age of 18 and unable to shop for and/or prepare meals for themselves due to a physical or mental health condition. Depending on need, preference, and location, clients may choose to receive either a weekly delivery of frozen meals or a daily delivery of a hot dinner and a cold bagged lunch. Meal cost depends on a client’s age, income, and expenses – sliding scale funding and a variety of other options are available.
All meals are delivered by volunteers, who pick up meals at our office Monday – Friday and deliver on an assigned route that takes about 1.5-2 hours and includes approximately 10-15 stops. Volunteers can choose to deliver as little as once a month or as often as every day. The application process is simple; we welcome individuals, organizations, and groups to help make a difference in the lives of our clients!
The Church of Saint Luke
The Parish Nursing Ministry of The Church of Saint Luke consists of two Parish Nurses and a mixed team of nurses, other health care providers, educators, and dedicated parishioners. Mindful of the connection between faith and health, the team reaches out to support those who are facing challenging health issues to provide support, comfort, prayer, and practical assistance. Assistance may come in the form of home visits, home communion, transportation assistance to medical appointments, individual office visits, help finding community resources, and bereavement support. Also, the team meets monthly to assess parish health needs and plan health education events. Although nurses are strongly encouraged to join our team, no specific education or background is required aside from a desire to serve God and the parish of Saint Luke.
